Not sure what to call the second and third chords, but this has got to be about right, was as close as I could get. You can figure out what to do with your right hand, and you can play with the fingering however you like I guess, but I found it pretty helpful to use a weird shape for the opening D major chord- rather than barring the top two notes with my first finger, I use all four fingers. First finger on the high string, second finger next to it on the B string, then third and fourth on the lower two of the chord. This way you keep your hand in place and just slide your first finger down to 9th then 8th fret on the high E string for the next two chords. This repeats in a mind-numbing loop, but you don't play the D chord twice when you repeat- the last D chord of the verse is also the first D chord of the next loop.
